Services 


Social Services

JHCHC considers behavioral health to be vitally important to the overall well being of patients. The social services staff works closely with the medical providers to make counseling services available to patients in a confidential setting, without the stigma of a “mental health center” setting. This promotes compliance and follow-up, both important factors in accomplishing overall patient wellness. Serious in-patient mental health needs are referred to the Mississippi State Hospital and other area mental health facilities. Substance abuse treatment is referred to Harbor House, Friends of Alcoholics and other area agencies.

ENABLING SERVICES

  • Case Management
    JHCHC employs the Chronic Care Model (CCM). This promotes the implementation and use of disease specific registries for the purpose of facilitating follow-up and case management especially among hypertensive, asthmatic and diabetic patients.
     
  • Eligibility Assistance
    JHCHC has on staff eligibility workers to assist patients in applying for Medicaid and the Children’s Health Insurance (CHIP) program.
     
  • Health Education
    JHCHC makes available health education materials on topics ranging from smoking cessation to teen sex and STDs. Health education is also made available through community outreach at community health fairs and special events.
     
  • Homemaker/Aide Services
    JHCHC may make referrals to a home health service that may, in turn, implement these services.
     
  • Nursing Home and other placement
    JHCHC may make referrals to a home health agency that may, in turn, make referrals, as needed.
     
  • Outreach
    JHCHC is a participant in the State Division of Medicaid’s Perinatal High Risk Management Infant Services System (PHRM/ISS). This program provides case management and follow-up services to pregnant women and infants who are eligible for Medicaid benefits and who are identified as “high risk” through such factors as teen mother, low birth weight, substance abuse, neglect, family violence, etc.
     
  • Community Development
    The Board of Directors, officers and staff of JHCHC recognize and accept the role of the health center as a good corporate citizen and are fully committed to the development of the communities that are served by us. Among this diverse group are elected officials, community activists and civic leaders.
